BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2013203396 DESCRIPTION A man in his 30s with a history of end-stage renal disease (ESRD) and metastatic parathyroid carcinoma presented to the hospital with severe shortness of breath, hypercalcaemia and diffuse bone pain. Chest radiograph (figure 1A) demonstrated profound demineralisation, and the distal phalanges appeared abnormal (figure 1B) due to bony resorption characteristic of severe hyperparathyroidism. Parathyroid carcinomas are an exceeding rare cause of hyperparathyroidism. It is important to distinguish between benign parathyroid adenomas and parathyroid malignancies, since the latter carries a very high mortality and the only successful treatment is surgical resection. 2 As with this patient, those with parathyroid carcinomas, unlike their counterparts with benign adenomas, tend to be younger, have ESRD or some degree of renal insufficiency, have severe hypercalcaemia (>15 mg/ dL) and present with intense symptoms. Radiological findings such as osteitis fibrosa cystica (figure 1A) are far more common in parathyroid carcinomas (44–91%) compared with benign parathyroid adenomas (5%) due to the degree of parathyroid hormone elevation and bone resorption seen in the former.

Utility of 99mTc-Sestamibi SPECT/CT in the Early Localization of Metastatic Parathyroid Carcinoma

Parathyroid carcinoma is very rare, with only a few documented cases. Hence, metastatic lesions are infrequently documented on scintigraphic imaging. We present a case of a 63-year-old female presenting with elevated serum levels of ionized calcium and parathyroid hormone (PTH) who was referred to our department for a parathyroid scan with SPECT/ CT. Parathyroid scintigraphy showed a focus of i...

The effect of a resistance training course with vitamin D supplementation on bone resorption markers in girls

Decreased bone density, especially in women, is a health risk factor, so the aim of this study was to investigate the effect of a resistance training course with vitamin D supplementation on bone Absorption markers in girls. Methods: The subjects of this semi experimental study were 24 non-athlete girls in Boroujerd (mean age 21.6 ± 2.04 years, mean height 155 ± 5.5 cm and average weight 64.13 ...

An analysis of the role of serum in parathyroid hormone-induced bone resorption in tissue culture.

THE stimulation of bone resorption by parathyroid hormone can be quantitated in tissue cultures of embryonic bone [20], using paired limb bones from a single embryo in which radioactive calcium (45Ca) has previously been incorporated. The ratio of 45Ca release from the parathyroid hormonetreated bones to that from the control bones is a sensitive measure of parathyroid effect. This technique ha...

Direct extraction and assay of bone tissue collagenase and its relation to parathyroid-hormone-induced bone resorption.

A method has been developed for the quantitative extraction of collagenase from as little as one 19-day-fetal-mouse calvarium. About 20-40 munits of collagenase are extracted per mg of tissue, all in a latent form that, after proper activation, shows the typical properties of mammalian collagenase. Culturing the calvaria for 2 days with parathyroid hormone (PTH) increases their procollagenase c...

Expression of parathyroid hormone-related protein confers malignant potential to mucoepidermoid carcinoma

Parathyroid hormone-related protein (PTHrP) is known to induce bone resorption by activating RANKL as well as PTH. PTHrP plays a central role in humoral hypercalcemia, and its expression has been reported to be closely associated with bone metastasis of breast carcinoma. PTHrP expression in oral squamous carcinoma cell lines was investigated, and PTHrP was expressed in oral squamous cell carcin...
